Hospice or Urgent Care

HOW IT WORKS:
Certify for qualified individuals, choose their Audio Bible language and translation (and/or other recorded materials), send us the signed order form and materials are sent by mail (FMFB - US Postal - Free Matter For Blind & Print-handicapped) at no cost whatsoever.

STEP 1: View & Print the Order Qualification Form. Be sure to include materials desired. You will find a list of all recorded materials here.

STEP 2: Have the order form signed by someone from hospice or the facility providing care. See a list of other types of certifiers here.

STEP 3: Send us the signed order form. To expidite the request, please write "HOSPICE" in large print on the order and fax to us at 941.748.2625.

Other Options:

• Contact the Hospice/facility and have them fax to us [941.748.2625] the following information on their orginizations letter head.
  • Patients name and address.
  • Their Name and position/title.
  • The desired Language - view our language/materials list
  • Choice of either cassette tapes or MP3 disc.
• Call us at 941.748.3031 [Monday - Friday, 9am to 5pm EST] and we'll help guide you through the process and help you get an Audio Bible as quickly as possible.
